What is your student dancers name? What grade is your student dancer in? What classroom is your student artist in? Who is their teacher at Fisler? Parent, guardian name, email, and phone number for emergency I give permission for all attendees and their art to be photographed by All the Arts for All the Kids Foundation to be used for marketing and on their social media platforms. If student's behavior at anytime time threatens the immediate safety of him/her ,other students, or staff, the parent/ guardian, will be notified and expected to pick up the child immediately. If the the unsafe behavior continues after the parent involvement, the child will be removed from the class permanently. A prorated class fee will be reimbursed, if applicable. As the guardian, I have read this policy and and agree with these terms. Anything we need to know to help us with any artists? allergies? Mobility? medical conditions? glasses? has a FSD Aide during the school day? ADA requirements? Mobility? Who is able to pick up your child from Dance class? If your child goes to an after school Lab/Encore, please advise that team of the change.
