Student Artist Name Student Artists Grade level Who is your student artists teacher during the day? Homeroom? Class number? If student's behavior at anytime time threatens the immediate safety of him/her ,other students, or staff, the parent/ guardian, will be notified and expected to pick up the child immediately. If the the unsafe behavior continues after the parent involvement, the child will be removed from the class permanently. A prorated class fee will be reimbursed, if applicable. As the guardian, I have read this policy and and agree with these terms. I give permission for my student artists and their art to be photographed by All the Arts for All the Kids Foundation to be used on for marketing and on their social media platforms. Who is able to pick up your child from this class? **If your child goes to Encore/LAB PLEASE advise that team of change of plans! Does your student artists havetheir own iPad they can bring to class. I give All the Arts for All the Kids Foundation permission to install an app that is district approved.
