What is your student dancer's name? What grade is your student dancer in? What school does your student dancer attend? ADULTS-just type the word adult :) Parent/Guardian name, email and phone number for emergency I give permission for my student artists and their art to be photographed by All the Arts for All the Kids Foundation to be used on for marketing and on their social media platforms. Yes No If a student's behavior at any time threatens the immediate safety of him/her, other students, or staff, the parent/guardian will be notified and expected to pick-up the child immediately. If the unsafe behavior continues after the parent involvement, the child will be removed from the class permanently. A prorated class fee will be reimbursed, if applicable.
